We're sorry to see you go. To cancel your membership at The Gym Lakewood, please submit the cancellation form below or contact us directly.

Cancellation Policy

A 21-day written notice is required to cancel your membership.
Your cancellation takes effect at the end of the next billing cycle after the 21-day notice period.
No partial-month refunds are issued.
Medical cancellations may be processed immediately with valid documentation.

After submitting your cancellation request online, you will receive a confirmation email. Please retain this for your records.
